The Yale Cardiology Network has developed ambulatory case management tools and reporting systems for its managed care contracts. The Yale Cardiology Network has strengthened medical center-based cardiology services by formalizing referral relationships with community cardiologists throughout Connecticut and into Rhode Island and New York. Yale New Haven Health System is pursuing future specialty network relationships and piloting VHA group purchasing programs and managed care initiatives through its Managed Care Network. Clinical planning, educational program enhancements and quality improvement YNHHS facilitates discussions on the development and expansion of residency, teaching and key clinical programs, as well as consultative service relationships and CME programs. We work together to explore systemwide clinical service lines that will improve quality, increase access and enhance physician communication and practices across the system. YNHHS also facilitates joint quality improvement initiatives among its physicians through a range of physician-led programs for both inpatient and ambulatory care. |
